Leadership for Safety Excellence (LSE)™ Proficiency evaluates participants' ability to apply classroom learning to practice. LSE™ Proficiency will become available in your Learning Centre after successful completion of the LSE™ course.  

LSE™ Proficiency is a mandatory component only for individuals enrolled in the NCSO® or NHSA™ programs.

• Optional for all other participants

Submission Requirements for All Four Documents

• Complete provided forms in Adobe Reader or Adobe Acrobat
• Submit together in a single PDF
• Text fields highlighted in red are mandatory
• Submissions with incomplete mandatory fields will be returned for revision
  The individual submitting the form must complete it
• Forms must include the name and electronic signature of the LSE™ Proficiency candidate
• Forms must be completed individually (for example: workers from the same company may not conduct an investigation as a group and then submit the same investigation for all participants)
• You may use workplace experiences for this assignment, but you may change names, titles, or use hypothetical scenarios
• Keep your work respectful and professional, as it will be reviewed by ACSA reviewers
